Medical Reports

Medical reports prepared by GPs are formal documents that provide an accurate, factual overview of a patient’s health. Commonly requested by third parties, these reports help inform decisions related to work, insurance, driving, legal matters, and other specialist assessments.

What These Reports Include

  • Purpose: To offer an expert medical opinion to organisations such as employers, insurers, and local authorities.
  • Content: A clear summary of the patient’s medical history, current health status, diagnoses, and any ongoing treatment plans.
  • Confidentiality: Reports are only shared with the patient’s explicit consent, ensuring full compliance with medical privacy standards.
  • Types of Reports:
    • Insurance and financial assessments
    • Employer or occupational health referrals
    • Fitness-to-work or fitness-to-participate certificates (e.g., sports, travel)
    • Private health assessments

Private Service

Most medical reports fall outside standard NHS services and are therefore provided as private, chargeable services.

We have provided a price list for the medical reports that incur a fee.

Our Medical Report Request form must be completed for every request to ensure we have all the necessary information and can follow your instructions accurately. It also serves as a reminder that some requests may carry a charge.

We accept payment by cash or cheque, which should be made at reception when you submit your completed request form.
